Marketplace Application Form

We ask that you fill out all portions of this form. Failure to do so will result in a delay of store production and inability to receive funds. Below are screenshots partnered with helpful hints to ensure every portion is filled out correctly.

Store Information

This portion of the form requests store contact information

  • Department Name: This will be what you want your store name to be and the informaiton that will show on the customers credit card statement
  • Department Number: You four digit department number
  • Contact Person: This is the name of the store contact that will be working with Cash and Treasury Services to build and maintain the store
  • Email Address: This is the email address of the contact person
  • Phone Number: This is the phone number of the contact person
  • If your store, department, or event has an existing website please enter it

Background Information

This portion of the application requests product/service information

  • Please provide us with as much information as possible. This will better assist our efforts to help you. This includes anything you believe we need to know about your store and how you are planning on using it. Examples of necessary information are provided above

Store Accounting Codes

This portion of the application requests account information

  • Please provide all chart field strings that your department will utilize to collect funds online. Inaccurate or incomplete information will result in a delay of store production. Along with the codes please indicate which code will go to which product. Please make sure that budgets are open for the chart field strings you are providing.

Merchant Fees Account

This portion of the application requests the account information for the credit card fee payment

  • Enter the Fund, Program, Department, Class and Project for the account code you want to pay the credit card processing fees associated with this Marketplace store.
  • The Account code is locked to Bank Fees and cannot be changed.

Volume Questions

This portion of the application requests estimated sales information

  • Annual Dollar Amount: Expected Revenue for the Year
  • Annual # of Transactions: Expected Number of Transactions for the Year
  • Average Dollar Amount per Transaction: Annual Dollar Amount divided by Annual Number of Transactions
  • Other Comments about Volume of Sales: Any Additional Information
  • Is a Peak Period of Activity Expected: Is there a time when a mass quantity of orders are to be expected?

Authorization

This portion of the form requires authorization signatures

  • This portion of the application asks you to sign, acknowledging that you have read the policies and completed the application to the best of your abilities. By signing and submitting the application, you are agreeing to the policies of Marketplace, outlined in the User Agreement located in the Forms page on this website
